Suicide prevention – aftercare

What is available?

Primary Health Tasmania has commissioned the Universal Aftercare service to provide evidence-based, trauma-informed psychosocial support to people who have recently experienced a suicide attempt or been in suicidal crisis.

The service includes:

  • rapid and intensive follow-up care
  • tailored practical support (housing, employment, relationships, education)
  • proactive outreach to reduce barriers and encourage engagement
  • collaboration with clinical services for safe and connected care.

It is delivered by suitably qualified mental health practitioners.

The Universal Aftercare service has evolved from and replaces the Way Back Support Service in Tasmania. 

Who is eligible?

People aged 16 years and older who have experienced a suicide attempt or suicidal crisis in the previous four weeks.

Where is it available?

This service is available to people living throughout Tasmania.

While it may not be physically delivered in every local community, eligible people have access to the service either face-to-face or through other means such as phone support. Speak to the service provider (below) for more information.

Who can make a referral?

Referrals can be made by:

  • GPs
  • hospitals and emergency services
  • mental health professionals.

Referrals are actioned quickly – eligible people are contacted within two business days of referral acceptance. 

How much does it cost?

There is no cost to people using this service, which is funded by the Australian Government under the Primary Health Networks Program.

Is there a related Tasmanian HealthPathway?

Yes. Health professionals can log in to the Tasmanian HealthPathways portal and search Suicide Prevention and Aftercare.
