This interactive portal aims to put you in touch with services commissioned by Primary Health Tasmania (Tasmania PHN) in your local area.
The map on the homepage shows the local government areas (LGAs) where particular services are available to local residents. Note not all services are physically delivered in each LGA indicated, but people in that area have access to the services either face-to-face or through other means such as telehealth or phone support.
You can search for particular services – go to the Help page for instructions on using the search function. Search results link to information including what the service covers, where it is available, who is eligible, referral processes, and contact details for the service providers.
Note this portal doesn’t cover all primary health services available in Tasmania – only those commissioned by Primary Health Tasmania under the Australian Government’s PHN Program.
